Same-day appointment for established patients who are ill and call before 9:00 am.
General and preventive medicine for adults.
Pediatric care, including well-child care. Age Limit Applies - Children over the age 12 years only.

Immunizations (childhood and adult) and chronic medication injections
Minor surgical procedures, such as skin biopsy or ingrown toenails

APPOINTMENTS
Consultation with Dr. Arnecilla is by appointment only.   Call us at (512) 377-AFFM to schedule an appointment.

Each time that you call the clinic, our staff confirms that our records have your correct daytime and evening telephone numbers as well as your correct mailing address. This will allow our clinic staff and physicians to contact you with test results and appointment information.

At each visit, please bring your insurance card with you and be prepared to pay your co-payment or deductible. Payment is requested at the time of your appointment.

Special Information for New Patients:

Please arrive thirty (30) minutes before your scheduled appointment to complete the registration forms.

APPOINTMENT CANCELLATIONS
 

If you are unable to keep your appointment, please call the clinic at least 24 hours in advance.

If you fail to keep your appointment with us on several occasions without calling the clinic in advance to cancel or reschedule, your doctor and the clinical staff will not be able to continue to provide medical services for you.

 

Calling to cancel and reschedule is important for your care and as a courtesy to other patients who may need urgent care.

NO CALL / NO SHOW POLICY
 

It is unfortunate that we must have a policy such as this for missed appointments. 

However, Dr. Arnecilla takes the time to listen to each patient's concerns.  When a patient fails to call and reschedule then that appointment might have been given to you when you called earlier for an appointment.

NO SHOW CHARGES will be applied to your account

when you fail to give at least a 24-hour cancellation notice.

The only acceptable method to cancel your appointment is by calling the office and speaking with the front desk staff or leaving a message on the General Voicemail Box.  Sending an email or fax is not acceptable.
Missed annual physical exam/Pap office visit   $50 No Call/No Show Charge
Missed routine office visit   DOUBLE the copay amount or $35, whichever amount is less

New Patients who fail to give at least a 24-hr cancellation notice

will not be offered the privilege of rescheduling the appointment.

EMAILS
Unfortunately, we are no longer able to provide this service effectively due to enormous volume of spam email.

CALL THE OFFICE for urgent medical situations - DO NOT USE EMAIL.

TELEPHONE CALLS
All messages are given to the appropriate person as soon as possible, and every attempt is made to return telephone calls during regular working hours. There are times, however, when we cannot answer all calls within the same day they are received.

Please tell us if your call is urgent, or must be returned the same day or by a particular time. Again, so that we might accurately reach you, please confirm your contact telephone numbers (daytime and evening) with our clinic staff.

TEST RESULTS
Ask your physician how and when you will be notified of test results. It takes approximately 14 days to receive Pap smear results or results of special tests sent out to the reference lab. If you have not heard from the doctor within 7 to 14 days, please call the clinic.

HANDICAP ACCESSIBILITY
We want to ensure that your visit to our clinic is as convenient and comfortable as possible. There are several handicap accessible parking spaces around all three entrances to the building. The building and surrounding area is a smoke-free environment. All building restrooms and elevators are ADA compliant. The clinic waiting area, restrooms, and examination rooms are ADA compliant as well.

PRESCRIPTION REFILL POLICY Back to top

AFFM is happy to provide prescription refills for our patients.  Please be aware of our prescription (Rx) refill policy.

  • Patients with chronic and stable medical conditions will receive enough Rx refills to total a 4 month supply.
     

  • No prescriptions are refilled for more than 1 year without an office visit.
     

  • A fee ranging from $5 to $10 per Rx is charged for Rx requests filled without an office visit.
     

  • No refills are done on Fridays, Saturdays, or Sundays. However, feel free to have your pharmacy fax the requests for our review Monday morning.
     

  • Other than the above mentioned exceptions, we answer all Rx requests within 24 hours during weekdays.
     

  • Under no circumstances are antibiotics prescribed without an office visit.
     

  • Please contact your pharmacy to have them fax a refill request to our dedicated fax line: 512-323-5256.
